Job Description:

CareConnect is seeking a compassionate Personal Care Specialist to provide exceptional care to patients in their homes. As a trusted member of our team, you will play a vital role in ensuring the safety and well-being of our clients.

### Key Responsibilities

* Provide personal care to patients, including bathing, dressing, and grooming
* Assist with mobility and transfers to prevent falls and injuries
* Administer medications as directed by healthcare professionals
* Monitor vital signs and report any changes or concerns to medical personnel
* Provide emotional support and companionship to patients and their families

### Qualifications

* Certified Personal Care Assistant (PCA) or Certified Home Health Aide (HHA) certification
* Understanding of HIPAA regulations and patient confidentiality
* Ability to cook simple meals and perform light housekeeping duties as needed
* Satisfactory completion of New York State Health Department-approved HHA Training Program or equivalency
* Demonstrated adequate literacy and simple arithmetic skills
