Job Summary:Aveanna Healthcare is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team in providing high-quality home health care services. As an LPN, you will play a vital role in delivering patient-centered care, collaborating with our interdisciplinary team, and making a positive impact on our patients' lives.Key Responsibilities:

• Deliver and coordinate skilled nursing care in accordance with physician orders and developed care plans
• Continuously observe and assess patient condition, reporting changes to supervisors and physicians as needed
• Document all activities, assessments, nursing actions, and patient responses in a timely manner
• Participate in and update the nursing care plan to ensure effective patient outcomesRequirements:
  • Graduate of an accredited school of nursing
  • Current, unrestricted state license as a Licensed Nurse in the state of practice
  • Current CPR certificationPreferred Qualifications:
    • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
